Understanding Membership-Based Practice

Proactive Path Medicine is a membership-based model where you pay your doctor directly, no insurance billing involved. Instead of a per-visit copay, you pay a flat monthly fee that covers your primary care. This removes the insurance layer from the relationship entirely, which means appointments are longer, access is faster, and the focus stays on you rather than on billing codes.

Not at all. Many members are in good health. The goal is to keep them there. On the proactive side, that means regular check-ins, advanced screening when it's warranted, and a doctor who knows each patient's baseline well enough to catch something early if it starts to shift. On the reactive side, it means that when something does come up — an illness, an injury, a question about a medication — members reach Dr. Chambers directly instead of sitting in an urgent care waiting room. Most non-emergency concerns can be addressed same-day, by phone, text, or video, whether at home or traveling.

Adults 18 and older. Some members are managing complex medical histories with multiple specialists and medications. Others are healthy and want to stay that way with a proactive, prevention-focused approach. What they have in common is that they want a doctor who knows them, has time for them, and is paying attention between visits.

Lab work and imaging are ordered by Dr. Chambers but processed through outside facilities. Members pay the lab or imaging center directly, often at lower negotiated rates than what insurance-based practices charge.

Dr. Chambers does not provide pediatric care. All members must be 18 or older. Dr. Chambers does not manage pregnancies. Members who become pregnant will need an OB-GYN as their primary pregnancy provider. Dr. Chambers does not prescribe chronic opioids or provide medication-assisted treatment for substance use disorders.

No, Proactive Path Medicine does not bill insurance. Members pay a flat monthly fee that covers their primary care. There are no claims, copays, or per-visit charges. You should keep your existing health insurance for specialist visits, hospitalizations, prescription medications, lab work, and imaging not covered under the membership.

Appointments and Access

Yes. Many concerns can be addressed by phone or video, whether a member is at home, at work, or traveling. Virtual visits are included in the membership at no extra charge.

Things come up. If you need to reschedule an appointment, just let us know and we'll find a new time. Membership itself is month-to-month with 30 days' notice required to cancel.

The first step is to schedule a free discovery call with Dr. Chambers. This is a chance to ask questions and make sure the practice feels like the right fit. From there, members sign a membership agreement and schedule their first comprehensive visit.
Communication and Specialty Care

Members have Dr. Chambers' direct contact information. For urgent but non-emergency concerns, she can be reached by phone or text, including evenings and weekends when appropriate. For life-threatening emergencies, always call 911 or go to the nearest emergency room.

Dr. Chambers coordinates the referral, communicates directly with the specialist, and stays involved throughout. She functions as a medical quarterback, making sure nothing falls through the cracks and that the care plan stays cohesive across providers.

Absolutely. Dr. Chambers is happy to work with any specialist a member already sees and trusts. She will also coordinate new referrals when needed and can recommend specialists she knows and communicates with regularly.
